IRS Form 1096 is the Annual Summary and Transmittal of U.S. Information Returns. This form is used to transmit Forms 1097, 1098, 1099, 3921, 3922, 5498 and W-2G to the Internal Revenue Service.
According to the instructions on the form, it is not used to transmit electronically. Only one type of form should be included with each Form 1096. For example, group all 1099 forms together and submit with one Form 1096. The entity filing Form 1096 is the same entity found in the upper left area of Forms 1097, 1098, 1099, 3921, 3922, 5498 or W-2G. Tax forms may be ordered from the IRS.
