In addition, a dedicated guest survey is available at Applebees.com/survey, which is accessible via computer and mobile devices.

Applebee’s feedback forms and surveys require information that can be found on the guest's receipt, such as the date, time, check number, total amount due, a unique nine-digit serial number and the server's name. As of 2015, taking the survey enters the guest in an instant-win sweepstakes that requires the entrant to be 18 years or older and a legal resident of the United States.
