For example, the Web page for Pennsylvania Nurse Aides has a Registration Application link. The New Hampshire Nurse Aides page has no application, but New Hampshire's Candidate Handbook offers a toll-free phone number to call for information on applying to take the test. Oregon is an example of a state that does not appear, so it does not have a nurse aide exam program.
