Where can you fill out a U.S. security card application?

The application for a U.S. Social Security card must be submitted in person or by mail to the local Social Security Administration Office. There is no online application available.

Whether applying for a replacement or original card, the applicant needs to provide original or certified copies of particular documents, depending on the circumstances. Applicants need to provide records, such as a U.S. passport, birth certificate or other documentation proving U.S. citizenship. An applicant also needs a picture identification to show identity. A U.S. driver's license can be used as proof of identity, as can an employee identification card or a state-issued identification card.
