According to a paper in Ophthalmic Plastic & Reconstructive Surgery, one method for reducing the epicanthic fold involves an upper-eyelid crease incision followed by a reshaping of the eyelid. This procedure is reported to be minimally invasive and required follow-up surgery in only 3 percent of the patients included in the sample population.
