To submit an invoice for services rendered, fill out a custom invoice form or template with the quantity of services rendered, the name of the service, and how much the service cost. Also note on the invoice the acceptable forms of payment and when payment is due.

Include the name, phone number and address of the business on the invoice form or template. If multiple services were rendered, list them individually on the invoice. Also note on the invoice whether the services were charged by the hour or by the individual service. List the grand total of all services rendered at the bottom of the invoice as well as to whom checks should be made payable.
