What is DD Form 2795?

DD Form 2795 is the Pre-Deployment Health Assessment Form used by the U.S. Department of Defense for all service personnel prior to deployment, according to the Deployment Health Clinical Center. Completion of the form is part of the pre-deployment activities required of all personnel and their families.
The DD 2795 must be evaluated and discussed during an in-person interview with an accredited health care professional, who can be a physician, nurse practitioner, physician assistant, Independent duty technician or Special Forces medical sergeant. Personnel and their families also are required to complete briefings, training, counseling and administrative actions in addition to the health assessment.
