What is Form 1099-R?

Form 1099-R is the tax document used to report distributions from retirement accounts, annuities, pensions, insurance contracts and profit-sharing plans, according to the Internal Revenue Service. As of 2015, a company must issue a 1099-R in each year that distributions or payments treated as distributions total at least $10.
Form 1099-R includes the payer's contact information and tax identification number, the recipient's contact information and taxpayer ID, the type of distribution, and the gross distribution amount, notes IRS.gov. The form also lists the amount that is taxable and the subtotals of federal, state and local tax withheld from the payment.
