What format should an addendum have?

An addendum to an existing contract should include an acknowledgement of the original contract agreement, the terms of the agreement's renegotiation, and any and all changes to the agreement. The addendum should be signed, initialed and dated by each participating party, and a witness, if the parties deem it necessary.
