The consent form asks for basic personal and medical information such as pre-existing medical conditions and primary care physician information. The questionnaire is used to determine which vaccines may be given at any visit. Sample questions include whether the patient is currently in good health, has received a vaccination in the past four weeks or has an immune system problems, as reported by Rite Aid.
