IRS Form 14039 is an Identity Theft Affidavit, according to the IRS. The form should be filled out by individuals who feel they have been a victim of tax-related identity theft. The form is filed with the IRS.
Tax-related identity theft occurs when another individual uses a stolen Social Security number to file a tax return and claims a fraudulent refund, the IRS explains. Usually a person uses the Social Security number early in the year, and the person with the stolen number does not learn of the identity theft until he goes to file his return. Taxpayers learning of identity theft should call local authorities and notify the IRS.
