Why do you need to sign a HIPPA form?

The law does not require you to sign a HIPPA form, but the law requires your health care provider to ask you to state in writing that you received the notice, says the U.S. Department of Health & Human Services. Most providers ask you to sign a form stating this.

Signing this form does not mean that you have agreed to any extra use or disclosure of your health records, states HHS. If you do not sign, your providers still can use and disclose your information as the law permits; however, they must keep a record of the fact that they failed to obtain your acknowledgement of receipt.
