When are you required to give a two-weeks notice of your resignation?

It is customary to give two weeks notice under most circumstances. Exceptions include a work situation where a union agreement or an employment contract call for more than two weeks notice.
A physically abusive boss, sexual harassment, unlawful withholding of wages and a work environment that is unsafe mentally or physically provide justifiable reasons for quitting a job without giving notice.
An in-person resignation with an immediate supervisor or boss is preferable, with the subsequent sending of a follow-up email and an actual resignation letter. A professional resignation letter discloses an employee's last day of work and declares the employee's gratitude to the company for the opportunities the company has provided.
