Do universities provide forms for requesting a tuberculosis skin test?

Most universities have all the required tuberculosis, or TB, and other vaccination forms on their respective websites, states Lewis University. Universities in the United States require all new and international students to have a number of vaccines.
Students need to obtain the proper forms from their university’s website or clinic and have it filled in by their doctor. It takes 48 to 72 hours for TB test results, according to the Centers for Disease Control and Prevention.
In order to determine whether a test is positive or negative, a health care professional must read the tests and document the results on a vaccination form, reports the CDC.
