According to the USDA, APHIS Form 7001 is a mandatory health certificate for pets leaving the United States. Pet owners who are moving or traveling internationally for extended periods of time usually seek this form.
The form must be taken to a USDA-accredited veterinarian. After an examination confirms the pet is in good health and meets the regulations of the destination country, the vet signs off on the certificate. The document is then forwarded to the respective state's USDA Veterinary Services office. After verifying the doctor is accredited and the information is accurate, the USDA endorses the certificate, enabling the pet to travel.
