Where can you get an application for a Canadian passport?

Canadian passport applications are available online at Passport Canada, passport.gc.ca or at any Passport Canada office. Canadians living in the United States may also obtain forms at any Government of Canada office based in the United States.

To obtain forms online, bring up the Forms page at Passport Canada, and select the type of form needed. Adobe Reader is required to access the forms, and users have the option of clicking a link to download the program at the top of the Forms page.

Using the renewal as an example, clicking on the Adult Simplified Renewal Passport Application brings up the blank PDF form. Either print it out as is and complete by hand, or fill it out on the computer and then print.
