Where can you find a dialysis center?

You can find a dialysis center by using the search tool on Medicare.gov or the Dialysis Center Directory on HealthGrades. Medicare’s tool allows searches by location, facility name and facility star rating. HealthGrades offers a directory of dialysis centers organized by city and state.

To find a dialysis center using the tool provided by the Centers for Medicare & Medicaid Services, enter a ZIP code or city and state combination into the Location box in the tool, and click Search. From the results, filter by star rating by clicking the minimum star rating desired from the right-hand side of the page. To use the HealthGrades directory, click on a city and state to see dialysis centers in that location.
