Can you fill out a Lifeline application form online?

Some companies that offer Lifeline service have applications available online. The Lifeline Support website has listings of participating phone companies in each state, along with a link to their online application if the company offers one.

To qualify for Lifeline, the customer must meet low-income guidelines. The application requires the customer to verify eligibility, most commonly through proof of participation in a federal assistance program, such as Medicaid or the National School Lunch Program. The customer must provide this documentation to the company, which may require mailing a copy of an award letter, before the application is approved. The individual Lifeline providers are able to provide the customer with more information if requested.
