Unsupported Medicare paid claims to Pos-T-Vac resulted in $4.2 million in overpayment during a period from January 2008 to December 2009, according to the U.S. Department of Health and Human Services. The auditors sampled 100 claims and found Pos-T-Vac lacked proof of delivery for half of the claims. Other documentation irregularities included unsigned physician approval forms and insufficient patient condition documentation.
