As of 2014, CMS-1500 is a form issued by the Centers for Medicare and Medicaid Services and used by health care professionals to request reimbursement for services provided to patients. This form is used for reimbursement from various government insurance plans including Medicare, Medicaid and Tricare.

The CMS-1500 is a standard claim form used by all non-institutional medical providers or suppliers to bill Medicare carriers and durable medical equipment carriers when a provider qualifies for a waiver of electronic submission of claims. This paper claim form is also used for billing certain Medicaid state agencies. CMS-1500 is available in various formats including single part, multi-part and continuous feed.
