Diabetics get sleepy when their blood sugar is high because the thickening of the blood from excess sugar slows circulation, keeping oxygen and essential nutrients from entering the cells, according to Diabetes Self-Management. High blood glucose levels can also cause fatigue when sugar causes inflammation of the blood vessels.

When this happens, monocytes, a type of immune cell, enter the brain and cause fatigue. It is also known that low sugar levels in diabetics can cause fatigue, reports WebMD. The lack of sugar in the bloodstream starves the cells of energy, thus creating fatigue for the diabetic. Balanced blood sugar levels help keep fatigue at bay.
