What is a DOT physical exam form?

A Department of Transportation physical exam form is a certificate valid for up to 24 months certifying that an individual is physically qualified to serve as a commercial motor vehicle driver. The certificate is issued by a licensed medical examiner and is required by the Federal Motor Carrier Safety Administration.

The medical examiner also may issue a certificate for less than 24 months if it is necessary to monitor a health condition that may affect driving ability, the FMCSA website states.

A medical examiner licensed to perform the DOT exam may be a doctor of medicine, a doctor of osteopathy, a physician's assistant, an advanced practical nurse or a doctor of chiropractic, FMCSA states.
