Compliance forms for the Health Insurance Portability and Accountability Act are documents that health providers use to ensure the confidentiality of patient data through efficient and appropriate records management. These documents include a Notice of Privacy Practices, Patient Authorization Form and an Accounting of Disclosures Form.

The Notice of Privacy Practices tells patients how the provider distributes and protects personal information according to HIPAA guidelines. The Patient Authorization Form obtains patient approval for sharing Protected Health Information within HIPAA regulations. The Accounting of Disclosures Form allows patients to request records of any distribution of their information, both inside and outside of the medical facility.
