Lake Michigan Credit Union, Saturna HSA and Alliant Credit Union offer health savings accounts with no set-up or maintenance fees. Several other banks offer HSAs with no set-up fees and waive maintenance fees if customers maintain minimum balances.

HSA Bank and Wells Fargo HSA waive the monthly account maintenance fee if the customer maintains a balance above $5,000, as of 2015. Bancorp HSA waives the monthly account maintenance fee if the customer maintains an account balance of $2,500 or $1,500 with a direct-deposit relationship.

Banks that do not charge a set-up or maintenance fee charge for other activities such as ATM withdrawals, paper statements and non-sufficient funds.
