What is a lead paint disclosure form?

A lead-based paint disclosure form is used by real estate sellers and landlords to indicate whether or not a property contains lead-based paint. The 1992 Residential Lead-Based Paint Hazard Act led to the use of this form to protect property buyers and children from the harmful effects of lead-based paint.
The Act requires landlords and sellers to give buyers or tenants an informational pamphlet about lead-based paint. They must also provide the disclosure form, which indicates any known presence of lead-based paint on the property. The disclosure must be signed and dated, attached to the contract and signed by both parties.
