To obtain a Department of Motor Vehicles medical certificate form, visit the Medical Examiner's Certificate page on the Federal Motor Carrier Safety Administration's website, and download the PDF version. You must print this certificate on card stock paper, available at office supply stores. Alternatively, some state trucking associations sell driver physical exam kits and forms on their websites.

Medical certification is usually a requirement of obtaining a commercial drivers license, and states require a driver to have a certified medical doctor fill out the certificate. The doctor performs the examination and checks the driver's vision and hearing, among other things. The doctor signs and dates the certificate and provides his medical license information for verification purposes. The examining physician checks off boxes for driver limitations such as driving within an exempt intracity zone.

The examining physician must be certified by the National Registry of Certified Medical Examiners. Uncertified physicians can go through the steps to get registered with the FMCSA. The driver must sign and date the medical certificate as well, and doctors usually keep a copy and return the original form to the driver. The bottom of the certificate contains an expiration date, and the driver must keep the certificate with him at all times.
