Medical certification is usually a requirement of obtaining a commercial drivers license, and states require a driver to have a certified medical doctor fill out the certificate. The doctor performs the examination and checks the driver's vision and hearing, among other things. The doctor signs and dates the certificate and provides his medical license information for verification purposes.
