How do you find Ontario court forms?

You can find Ontario court forms at the Ontario Court Services website, OntarioCourtForms.on.ca. You can download forms related to topics such as family law, small claims court and civil procedures.

Most of the forms are available in both PDF and Microsoft Word format. Tables list the forms with form numbers and form titles. The lists include version dates to identify when the forms were published and effective dates to identify when the forms were put into use. Older, outdated forms are in the forms archive. You cannot file the forms electronically, so you must print, fill out and manually file them with the courts, according to the Ontario Courts Services.
