What is OSHA Form 301?

OSHA Form 301, Injury and Illness Report, is a form that employers must fill out to document work-related illnesses and injuries. The form, when used with OSHA’s Log of Work-Related Injuries and Illnesses helps the employer and OSHA to determine the extent of a specific workplace’s work-related incidents.

OSHA requires that employers fill out a Form 301 within seven days of learning of a work-related illness or injury. In some states, an insurance, workers’ compensation or other form may be substituted by the employer for the OSHA 301. Employers must keep the form or its equivalent on file for five years following the year the employer fills out the form.
