What is an accident report form?

The Oregon Department of Motor Vehicles says an accident report form is a form that must be filed with the Department of Motor Vehicles following an accident. In Oregon, the form must be filed within 72 hours; however, requirements may vary by state.

The Oregon DMV advises that the accident report form must be filed even if a police report is also being filed. The Accreditation Commission for Traffic Accident Reconstruction, a non-profit professional organization that serves accident reconstructionists, provides links to accident report forms and relevant legal codes on their website for states and Canadian provinces. In many cases, these forms can also be found on a given state's DMV website.
