When should an employer complete an OSHA accident report form?

An employer must complete an Occupational Safety & Health Administration report any time a work-related injury, illness or fatality occurs, as outlined in the OSHA Injury and Illness Recordkeeping and Reporting Requirements on the OSHA website. Fatalities must be reported directly to OSHA within eight hours.
Any injury or illness occurring as a result of an event or exposure in the work environment is considered work-related under OSHA regulations, the agency states. This includes pre-existing injuries or illnesses aggravated by an event or exposure in the workplace. OSHA defines the work environment as anywhere employees work or are required to be as a matter of their employment.
