Aflac's wellness benefit claim form is available online by visiting Aflac.com, clicking on Enter under the Individuals and Policyholders heading, and then clicking on Claim Forms. Under the Download A Claim Form heading, users select a state from the drop-down menu and click Submit.

Users select a wellness benefit claim form from the available choices, and click on the corresponding link to download the document.

Users complete, sign and date the form, and mail it to American Family Life Assurance Company of Columbus (Aflac), ATTN: Claims Department, 1932 Wynnton Road, Columbus, GA, 31999. Users can also fax the form to 877-442-3522, as of 2015.
