Are Aetna claim forms available online?

As of 2015, Aetna claim forms are available online from the company's website. Aetna.com offers various claim forms for free download. Users do not need to log in to access and download the claim forms.

On the Aetna.com main page, click on the Individuals & Families icon. A drop-down menu appears containing various sub-menu. Click on the Find a form category located on the For Members sub-menu. The site directs users to a new page containing various types of claim forms. Select the desired form to proceed to download. An Aetna claim form is only valid if an individual is a patient of the Aetna organization.
