Medicare can be reached by mail at Centers for Medicare & Medicaid Services, 7500 Security Boulevard, Baltimore, MD, 21244-1850. If a third party, such as a friend or family member, assists a covered individual by contacting the agency, the "Medicare Authorization to Disclose Personal Health Information" form has to be filled out and mailed to the address on the form before any confidential information can be discussed.
