Where can you find a printable DS-82 form?

Printable DS-82 forms are available at Travel.State.gov. Site visitors can print a PDF version of the form and fill it out by hand, or fill it out online and print the completed form.

The DS-82 form is an application for a U.S. passport that is submitted by mail. To use this form, applicants must submit their most recent passport. The passport issue date must be within the last 15 years, and it must be after the applicant's sixteenth birthday. Applications also require a fee, a photo and documentation of any name change.

Applicants who don't meet the requirements for the DS-82 form must apply for a passport in person using the DS-11 form, also available at Travel.State.gov.
