Where can you find a sample of an employee write-up?

Samples of employee write-ups can be found on About.com and Entrepreneur.com. A selection of blank write-up templates that may be filled out on the computer and then printed can also be found on TidyForms.com.

Employee write-ups should contain the name of the employee and the supervisor issuing the reprimand, as well as the date that the write-up was issued. It should also contain space to briefly describe the nature of the employee's infraction and the consequence decided on by the supervisor. Some write-ups include a space for the employee to write a statement about the infraction. All write-ups should include room for employees to sign, acknowledging that they read the form.
