The Wisconsin Immunization Registry is for the use of patients or their guardians in the state of Wisconsin. Patients or their guardians can use the registry to determine which immunizations patients have received and when.
Patients or their guardians can find the patients' immunization records by entering their first and last names and birth dates. The patient's Social Security number and medical ID or health care member ID is necessary to gain access the patient's records. Health care professionals are responsible for entering the data into the system. Free training is available for health care professionals by the state of Wisconsin.
