What is an exit interview form?

An exit interview form is a form an employee fills out when he or she leaves the company. It is used to determine why the employee is leaving, what he or she liked about the job and where he or she feels the company can improve.
Exit interview forms are generally used in one of two ways. They can be given to the employee on his or her last day or mailed to the employee's home. Exit interview forms are not always effective because not all employees return them and it takes a long time to compile data. However, these forms are often preferred because an employee is much more likely to reveal negative aspects of the position when he or she doesn't have to do it personally.
