Where do you fill out a Social Security appeal form?

As of 2015, the Social Security appeal form is available to fill out online in the Appeal Our Recent Medical Decision section of the Social Security Administration website. Applicants can either begin a new appeal or return to a previously saved appeal from this location.

Applicants who receive denials of their applications for Social Security disability benefits or Supplemental Security Income can appeal the decision. There are four levels of appeal, beginning with reconsideration of the initial decision. The levels progress to a hearing by an administrative law judge, a review by the Appeals Council and a Federal Court review.
