What documents are needed for a passport?

In order to obtain a passport, the application process requires evidence of U.S. citizenship, photo identification, a passport photo and Form DS-11. A certified U.S. birth certificate, a previous U.S. passport, a consular report of birth abroad or a certificate of naturalization/citizenship is acceptable as evidence of U.S. citizenship.

Acceptable forms of photo identification include a valid driver's license, an undamaged U.S. passport, a certificate of naturalization, a valid government I.D. or a valid military I.D. A passport photo must be 2 inches by 2 inches and in color, and the photo must have been taken within the past six months.
