What is an IRS payment voucher and how do you use one?

Form 1040-V is the IRS payment voucher. It is required of certain taxpayers to pay their balances. If a taxpayer is required to submit a 1040-V, it must be returned along with the tax return and payment.
The new payment voucher is used by the IRS to streamline its payment system. They can be obtained from local IRS offices or by visiting the IRS website. If the return was filed electronically, the payment and payment voucher are returned to the lockbox address associated with the service center where the return was filed. If the return was mailed in ahead of the payment, the payment and voucher are sent to the appropriate address located in the instructions.
