The opposite of pigeon toed, or in-toeing, is called out-toeing. Both in-toeing and out-toeing occur in infants and children and are usually a normal variant.

Out-toeing is uncommon and occurs far less frequently than pigeon-toe. When children who have external hip rotation contractures begin to walk, out-toeing may occur if the contractures resolve slowly. Normally, out-toeing still dissipates within the first year the child begins to walk.

Tibia and femur twisting can also cause out-toeing. However, these causes are usually linked to neuromuscular abnormalities. If this causes too much difficulty in walking for the child, the bone can be surgically cut and rotated to correct the problem.
