How do you replace lost AKC papers?

Dog owners can request for replacement registration certificates by filling out a form on the American Kennel Club website. Only recorded dog owners can obtain registration certificates, and they must provide their dog’s name and registration number, as well as a delivery address.

The form to request a replacement registration certificate is only available in PDF format. Dog owners who can't print the online request form can contact the AKC Order Desk to have a form sent to them. Customer service representatives are also available via phone and email to assist dog owners in the process of requesting and obtaining replacement registration certificates.
