File fraud reports online at VerizonWireless.com by entering "Fraud Report" into the search field at the top right of the home page and then selecting Fraud Prevention Verizon Wireless. The Continue to Secure Claim link at the bottom of the page directs users to the claim form.

Consumers filing a fraud report must provide proof of identification, a Social Security number, a copy of the filed police report and proof of residency during the time of the event. Business and corporate customers filing fraud reports must provide proof of business identity, a valid business Tax Identification Number, proof of the business address for the time the event occurred and a copy of the filed police report.
