The teacher and parents or legal guardian of the student are responsible for creating a 504 ADHD plan in schools. If the student is not a minor, they may self-advocate. School specialists and counselors may also be involved in writing a 504 plan.

A 504 is different than an Individualized Educational Plan (IEP) and is used to help students without IEPs receive accommodations so that they can be successful in a regular classroom. 504 plans are usually written by a team consisting of the student's legal guardian, teachers, any counselors or therapists who work with the student and the school administration.
