Sedation is required for an upper GI endoscopy, according to WebMD. First, the doctor sprays the patient's throat to suppress his gag reflex. Then, the doctor gives the patient a sedative and pain medication intravenously to keep him still and comfortable during the procedure.

The doctor may also give the patient a mouth guard to protect his teeth when the endoscope is inserted, says WebMD. The patient remains awake and aware enough to follow the doctor's instructions, such as to swallow the tube to help ease it down his esophagus. A suction device may help remove saliva from the patient's mouth, and the doctor's assistant may change the patient's position now and then to make the procedure easier.
