Where can you fill out a MassHealth application?

Massachusetts has the application for MassHealth on its website, at mass.gov. It is available either as an online application, or as a printable form that can be mailed in to the Massachusetts Health Insurance Processing Center.

To find the MassHealth application from the Mass.gov homepage, click on the Health & Safety tab near the top of the page. On the Health & Safety page, find the Most Requested section at the right and click MassHealth. On the MassHealth page, click the Apply for MassHealth link, then either apply online using the link near the top of the page, or download and print one of the PDF forms farther down the page.
