Is a downloadable W-9 tax form available as a PDF?

The Internal Revenue Service offers Form W-9, Request for Taxpayer Identification Number (TIN) and Certification, in printable PDF format on its website, IRS.gov. The form’s instructions are available in HTML format on the site.

Taxpayers use Form W-9 to provide their taxpayer identification numbers to the person or entity who files information about income or mortgage interest paid, real estate transactions, debt cancellation, acquisition of abandoned property and IRA contributions, notes IRS.gov. To find the form, taxpayers click on Forms & Pubs from the IRS.gov home page and then click on the link to Form W-9 from the Individuals section under the Current tab.
