A feedback form is a form that a user can fill out to describe his customer experience. It is commonly available on both websites and in brick-and-mortar retail locations.
On websites, these forms can often be used to elicit information from users. Sometimes, the information on the form is not emailed but rather posted somewhere on the site for other users to read, says About Tech. The feedback form is comprised of two types of code: the first part is an HTML source code that renders the form itself, and the second part is the submission code that sends the form out to the relevant parties, states Iron Spider.
