What are the requirements for a passport renewal application?

Passport renewal applications for the United States are submitted by mail, and require that the applicant's most recent passport is undamaged, issued within the last 15 years, issued when the applicant was 16 years or older and issued in the current name of the applicant. If the applicant's name has changed since the last passport was issued, an evidence of name change, such as a marriage certificate or government issued document, must be submitted with the application.

Applicants who do not meet these requirements cannot submit their renewal applications through the mail; they must make an appointment to apply in person at a local authorized passport acceptance facility. Renewal applicants must include a completed DS-82 form, one passport photo and the applicable fee in the form of a personal check or money order payable to the U.S. Department of State.
