What forms do you need to complete to renew a Guyana passport?

To renew a Guyana passport, an applicant must complete the Form A - Application for a Guyana Passport form and return her existing passport with the form. If the previous passport was lost or stolen, the applicant must complete the Lost or Stolen Passport Information Sheet section of the application.

In addition to the application form, an applicant must supply a suitable color photograph of herself as well as the relevant proof of her identity and her status as a citizen of Guyana. Those who were born outside of Guyana must provide proof of naturalization or other proof of current citizenship.
